Since there is no official report of the accident, a mistake since we learn so much more from "misadventures", we have to rely on scuttlebutt for info this. I have heard that some check valves were in back to front, valve spindles lacked packing, flanges in watertight bulkheads had no gaskets and leaked. And, of course, all crew were evacuated, surely this is a cardinal error. Does anyone have a formal report issue by the cert authortiy ABS?
